BEAR form (Billed Entity Application Form) submissions:
We have greatly simplified this step of the process. Submit your
BEAR to Tek Data via fax, attention Randy Kick at 1.847.367.0235,
it will be signed and returned to you promptly.
Please state the invoice number and/or the purchase order that
this form will be paid on. Include your contact information and
fax number on the cover sheet to eliminate any delays in returning
the signed form to you.
How to submit an E-Rate order (using the PO):
1. The purchase order must be clearly marked E-Rate
2. The total dollar amount of the PO must be submitted.
3. Please note the SLD's portion and school's portion in the
body of the PO.
4. Terms: 30 days
You must also submit the following forms with the Purchase Order:
1. Funding Commitment Decision Letter
2. Funding Commitment Report (FRN)
3. Description of Service (submitted with Form 471)
4. Signed copy of Form 486
